Case Manager  

Are you interested in having a positive impact on people's lives?  As a Service Facilitator, you are responsible for monitoring the services provided to individuals on Medicaid Waivers (Long-term care) the CCC+, CL & FIS waivers as well as EPSDT program recipients to ensure their safety, satisfaction, health, and welfare while living independently with support in the community.

Skills and Qualifications for a Service Facilitator (Case Manager) include:  

  • Requires a documented combination of skills/relevant work experience. Person-Centered Thinking/Planning training is a plus. 
  • Work remotely with electronic devices and associated tools and equipment required to complete day-to-day tasks. 
  • Maintains experience in a variety of applications and programs; not limited to but including Office, Google, & Adobe. 
  • Must maintain a steady and reliable internet connection. 
  • Must hold a valid driver's license and reliable transportation
  • Have good written and verbal communication skills. 
  • DMAS requires Service Facilitators (Case Managers) to have one of the following: An associate's Degree or higher in a human services field, a degree in a non-human services field, and a minimum of 2 years of relevant experience.
  • Submit a criminal background check. 
  • Two references are required.

 

Clinical Manager
The Clinical Manager directs the operations of Services. The Clinical Manager supervises a team comprised of a nurse and generalist professionals. The team is responsible for providing services to consumers who are eligible or enrolled in the Medicaid Waiver program.  The Clinical Manager coordinates service levels and medical assessments.   

Direct Support Professional-In-Home Care

A respite care aide will provide care in the home of the consumer, enabling the family caregiver to have free time.

Responsibilities:

  • Assistance with or prompting client regarding light personal hygiene;
  • Provide a safe environment for the consumer;
  • Provides companionship in the consumer’s home;
  • Medication Management;
  • Medical Appointments Linkage;
  • Provides Community Outings;  
  • Meal preparation; and
  • Light housekeeping.

Knowledge, Skills, and Abilities:

  • Be at least eighteen (18) years of age;
  • Be acceptable to the client;
  • Be able to walk in the community with the client; 
  • Be certified in cardiopulmonary resuscitation (CPR) and thereafter obtain CPR certification annually;
  • Demonstrate that he or she is free from communicable disease as confirmed by an annual PPD Skin Test or documentation from a physician stating that the person is free from communicable disease;
  • Have the ability to communicate with the client;
  • Be able to read and write the English language;
  • Have a high school diploma or general equivalency development (GED) certificate;
  • Be able to recognize an emergency and be knowledgeable about emergency procedures;
  • Agree to carry out the responsibilities to provide homemaker services consistent with the client’s IHP or ISP;
  • Complete Phase I and II orientation training approved by DDA;
  • Meet the minimum training and qualifications of a personal care aide, as set forth in Section 938 of Chapter 9 of Title 29 DCMR.

 LGSW/LGPC/LICSW
Therapists are responsible for providing structured, ethical, and culturally sensitive clinical counseling in accordance with the Virginia Department of Social Services. Provides counseling, reviews, and assessments for individuals served.
